Earache (acute otitis media) is a common condition, particularly in children, that can cause significant discomfort, irritability, and difficulty sleeping. It often develops after a cold, flu, or throat infection when fluid becomes trapped in the middle ear, leading to inflammation or infection.

In many cases, earache is painful but mild and improves within a few days. However, early assessment is important to ensure the right treatment is provided and to help prevent the condition from worsening.

The NHS Pharmacy First earache service is available for eligible children aged 1 to 17 years who meet the NHS eligibility criteria. Our pharmacist will confirm whether this service is suitable during your consultation.

Through the NHS Pharmacy First service, eligible children can receive timely assessment, professional advice, and treatment directly from the pharmacy, helping to reduce waiting times and improve access to care.

What Causes Earache?

Earache is most commonly caused by a middle ear infection (acute otitis media), which often develops following a cold, flu, or other upper respiratory infection. These illnesses can cause swelling and fluid build-up behind the eardrum, leading to pain, pressure, and inflammation. Earache can also occur due to blocked ears, excess earwax, changes in air pressure, or irritation of the ear canal. Children are more prone to ear infections because the tubes connecting the middle ear to the back of the throat are smaller and can become blocked more easily.

Common Symptoms of Earache

Common symptoms of earache include ear pain or discomfort, difficulty sleeping or irritability (especially in children), a mild fever, reduced hearing or a blocked sensation in the ear, fluid or discharge from the ear, and tugging or rubbing of the ear in younger children. Some children may also appear less active than usual or have difficulty feeding due to discomfort.

Treatment may include self-care advice, pain relief recommendations, and, where clinically appropriate under the NHS Pharmacy First service, the supply of prescription-only medication without the need to see a GP.
